Phr app shows could not find user details

Hello team,
I have register patient with EMR webapp with facility of Archana Eye Clinic and another with Kidney Center Hospital and assigns op consultation and upload dignostic report . When I am trying to linked this with PHR app it shows “could not find user details.”

https://emrsbx.ndhm.gov.in/ i am using this url to login in Emr web but currently today it shows internal server error. https://sandbox.abdm.gov.in/docs/phr_app and using this i have install phr app currently i am using it.

Not resolve yet.will you please help me to let me know.how to resolve it.
