PHR App functionality terms and conditions (agreements)

In multiple places in the PHR app, the user has to tick(or check) a check box that says that the user is willing to agree to terms and conditions.

As you can see the " I AGREE AGAINST PERSONAL DATA PROCESSING CONSENT FORM AGREEMENT" part needs to be checked . Where are the agreements available (i.e content of these agreements needs to be displayed before user agrees and checks the box), so where do we find these agreements.

This is on the app I am currently developing . I don’t have the content which is why I’m asking.
