Open Sourcing PHR reference app

Hey,

We were wondering if NDHM could open source the reference implementation of the PHR app.

The documentation has been a bit too complex for us to comprehend, open sourcing the PHR app code will help everyone to look at the source code understand the data flow, and will help in fast-track third-party development as we can build on top of the reference app.

I can see one project Jataayu, but it seems to be outdated.
