Sandbox PHR app crashing

Hi Team,

PHR app for sandbox keeps crashing every now and then during OIDC login redirection sometimes it gives error “Failure something went wrong” post redirection on app (refer to attachment) or sometimes it wont redirect and browser is stuck at the below address -
https://dev.ndhm.gov.in/oidc/?scope=profile&state=13-232424-ads-23d&action=login&accessCode=f7ead6a8-d547-4690-9a44-10f9aac913cf

I am also facing the same issue getting error “Invalid OIDC Transition” on phr app.

We tried to login with some users and it worked.
Try to keep details handy and then try.
