PHR App crashes

Hi @mdubey @sukreet

Hope you all are doing well,

PHR App is crashed when we are trying to see list of provides and list of Requests.
This error was come with some specific users,

abc6@sbx, bhupendra@sbx

There is one more issue while adding to care-context if we add without care-reference , care context was linked then if we want to accept the consent then PHR app throws error.
